Pop-up Vaccine Clinic at PS 125 Tuesday, November 9th (7:00am - 11:00am)

What we know is that MedRite expects to begin administering the vaccine at 7:00 a.m. and that they have a certain amount of Pfizer doses allocated for our site. If they run out, then families will be redirected to a different pop-up vaccination site. If our campus has a surplus supply, then there is a possibility we will offer those doses to schools that have run out of doses. Vaccines will be administered on a first come basis.

Please have the health screening form for yourself and your child ready when you arrive. If possible, print and complete this consent form before arrival.
